The following forms are available for current Upward Bound students for reimbursement purposes.

College Visit Travel Reimbursement
Funded through our partnership with John Deere
Available for juniors and seniors

To claim reimbursement:

  1. Inform UB staff before you are taking a trip that you would like to use reimbursement funds
  2. Print form or grab a copy from UB staff at your school
  3. Complete the form, including a signature from admissions staff at the college you visited
  4. Submit the form to UB staff within 30 days of travel
